Salon intake forms, filled in while clients book

Avana lets clients answer your consultation questions and accept your consent statement while they book online, per service. It's in every Avana plan, so you know about an allergy or sensitive skin before they arrive.

Why ask in advance

You know before the client walks in

Finding out about an allergy at the chair is too late. With Avana's consultation form you know before the appointment starts.

An allergy isn't news in the chair

Someone allergic to lash adhesive or tint tells you while booking, so you can adapt the treatment before you start.

Consent in writing

Your consent statement sits in the booking screen, and without the tick the booking doesn't go through. Avana keeps the text and the moment of consent with the appointment.

No more clipboard at the desk

Clients fill it in at home on their own phone. You start on time, and the answers are still there next visit.

Filling it in

From your question to the answer at the appointment

In Avana you set up the form on the service it belongs to. A lash lift gets different questions from a haircut.

Your questions

Add your own questions to a service

For each question, choose open text, yes or no, or multiple choice. With multiple choice clients can tick several answers, with a final option like "none of these apply to me".

  • Different questions per service
  • Open, yes or no, or multiple choice
  • A notable answer stands out
While booking

Clients answer and consent in one go

After choosing a time, clients see your questions with your consent statement underneath. If consent is required, they can't book without ticking it.

  • On the client's own phone, at home
  • A tick is the consent, no signature
  • Questions themselves can be skipped
In the confirmation

Clients get their answers and your document

The answers appear in the client's confirmation and in the email you get about the booking. If you've added a document, such as information about the treatment, it's attached to the confirmation.

  • A PDF, Word file or image up to 4 MB
  • Clients can check what they filled in
  • You see it straight away with a new booking
At the appointment

Read everything back before you start

The appointment shows the form with every question, every answer and the moment of consent. The client card lists the answers from every earlier appointment.

  • See the statement exactly as the client saw it
  • Editing the text doesn't change past appointments
  • Keep a signed paper form on the client card
Before and after the treatment

From booking to aftercare, everything runs through the same service. This is the step where clients tell you what you need to know.

Questions and the consent statement appear when clients book online themselves. If you create the appointment in your calendar, ask in the salon; a signed paper form can be kept as a document on the client card.

Before the appointment

The form is tied into the rest of your salon

Three parts of Avana you'll find on the same service or the same client.

QuestionsAftercare email

Your questions before, your tips after

On the same service you set up the aftercare email. What you asked beforehand, you know at the treatment; what the client needs to do at home follows on its own.

QuestionsClient card

The client card lists the answers from every appointment alongside your notes and photos. For a regular, you see straight away what they filled in before.

Consent and a deposit in the same booking

For a long or expensive service, ask for a deposit and consent together. The client reads your statement, ticks it and pays the deposit in one go.

Questions

Intake forms in Avana

How do I set up a intake form for salon clients?

In Avana, open the service and go to its questions. Add a question, type it and choose open text, yes or no, or multiple choice. If you also want consent, switch on the consent statement, write your text and save the service.

What kinds of questions can I ask?

Avana's consultation form supports open questions, yes-or-no questions and multiple-choice questions. With multiple choice, clients can pick one or more answers, and you can make the last answer exclude the rest for something like "none of these apply to me". Asking clients to upload a photo isn't possible in the questions.

Can I make a question compulsory?

No, questions are never compulsory, so clients can book without answering everything. If clients genuinely must agree to something, switch on the consent statement with consent required; without the tick the booking won't go through.

Do clients have to sign a consent form when booking online?

No, there's no signature. Clients tick the consent statement, and Avana keeps the text with the moment of consent on the appointment. If you change the text later, older appointments keep the version the client agreed to; if you still have a signed paper form, keep it as a document on the client card.

Where do I see a client's answers?

Avana shows the answers on the appointment in your calendar, with every question, answer and the moment of consent. The client card lists the answers from all appointments, and they're also in the email you get about the new booking; see client records.

Do clients get the questions if I book the appointment myself?

No. In Avana, questions and the consent statement only appear when clients book online themselves. If you create the appointment in your calendar, ask them yourself; no separate email goes out to fill it in later.

Does it work for lashes, skin and massage?

Yes, Avana's consultation form suits any treatment. For a lash artist you ask about sensitive eyes and adhesive allergies, for a beauty salon about skin type and previous treatments, and for a massage practice about complaints and areas to avoid. You set it per service, so only where it's needed.

Is the intake form included in every Avana plan?

Yes. Questions and the consent statement are in every plan, including Avana Business, with no charge per completed form. The pricing page shows what each plan includes.

Is there a service fee, and who pays it?

When a client pays online, a small service fee is part of the price they see from the very first screen: 3% of the booking, at least $1.99 and at most $2.79. You pay nothing on your takings, because Avana charges no commission. That small amount from the client is what lets us pay you out in full, so the whole treatment price stays with you.

The short answer

How do clients fill in a intake form while booking?

Avana is a booking system where clients answer your intake form questions and accept your consent statement while booking. You set up your own questions per service: open text, yes or no, or multiple choice. Underneath you add a consent statement with a tick, and choose whether clients can book without consenting; a document such as treatment information is attached to the confirmation. Clients fill everything in after choosing a time. You read the answers and the moment of consent on the appointment, and the client card lists them per appointment. Questions and the consent statement are in every plan, so Avana Business costs $20.99 a month, tax included, which is $0.69 a day.

Intake forms for salons

A consultation that's done before the client arrives

A salon intake form makes sure you know what you need to before the treatment: an allergy, sensitive skin, a previous treatment or a wish. In Avana you add those questions to the service they belong to, and clients answer them while booking on your booking page or in the widget on your site. Nothing to print, scan or leave on a clipboard.

If clients need to genuinely agree to something, such as the risks of a treatment or what to do beforehand, add a consent statement. They tick it on the same screen, and Avana keeps the text with the moment of consent on the appointment. If you also take a deposit for a long service, that happens in the same booking, and the answers appear in the confirmation, on the appointment and on the client card.

The form is the start of a line that runs through one service: your questions beforehand, the answers in your calendar at the treatment, and the aftercare email with your tips afterwards. It's all set up once in your services. It comes with every plan, as the pricing page shows.

Which questions belong in a beauty salon intake form?

Anything that could change the treatment or make it risky: allergies, skin type, medication that makes skin more sensitive, pregnancy, recent sunburn and previous treatments such as peels or injectables. Keep it short; five to eight questions is plenty. A long list gets half filled in, and then you still don't know.

Use yes-or-no questions where you can, because they're quickest on a phone. For allergies, a multiple-choice question with the usual suspects and a final "none of these apply to me" works well; if a client chooses anything else, that question is highlighted on the appointment so you don't miss it. Keep an open question for their wish: what would they like to see after the treatment?

Do you need a consent form for lashes or microblading?

For treatments with some risk, such as lash extensions, a lash lift, microblading or a chemical peel, it's sensible for clients to agree beforehand to what can happen: an allergic reaction to adhesive or pigment, redness, or a result that depends on aftercare. What exactly to include varies by treatment and by state, and your insurer or licensing board can usually point you to an example.

In Avana that text goes on the service as a consent statement. Clients read it while booking and tick to say they've read and agree to it; with consent required, nobody can book without the tick. Avana keeps the text with the moment of consent on the appointment, so even if you update it later, you can see exactly what the client agreed to then. A longer document, such as your full terms, can be attached to the confirmation.

Is a tick enough, or do clients need to sign?

For most salon treatments, a tick with the text and the time of consent is a clear record that the client read the information. The big advantage over paper is that you know exactly which text they saw and when, and it can't get lost in a folder. If you're unsure for a particular treatment, ask your insurer what they expect.

If you still use a signed form, for example at a first appointment, the two can sit side by side. The client accepts the statement online in Avana, and you photograph or scan the signed paper in the salon and keep it as a document on the client card. That way everything about the client is in one place: forms per appointment, consent on the appointment and the signed paper.
